DOCUMENTS NEEDED TO REGISTER A NEW STUDENT: If your child is re-enrolling at O'Callaghan M.S. from another
| 1. | One Proof of Address. One form of proof of address is required. You may use any of the following: 1) rental agreement, 2) mortgage statement, 3) sales contract, 4) power bill, 5) water bill or 6) gas bill. The parent/guardian name and the current address must appear on these documents. [Driver’s license, telephone bill or cable bill are not acceptable proofs of address.] |
| 2. | Personal Identification. Driver’s license, picture I.D., etc. (ID must bear your name and current address). |
| 3. | Withdrawal Paperwork from previous school. Student must be withdrawn from previous school before we can enroll the student here. |
| 4. | Notarized Residential Affidavit. If you are living with a friend or relative, you must bring a notarized statement signed by you and the owner/renter of the property where you are staying, indicating that it is your place of residence. A form may be picked up at the Registrar’s office. |
Students new to
| 1. | Immunization records. Prior to the student reporting to school for the first day, we must have immunization records indicating that your child has had, or at least has started the following series of immunizations. DPT shots, polio shots, MMR shots, Hepatitis A, Hepatitis B and Chickenpox (Varicella) shots. |
| 2. | Proof of the child’s identity. This may consist of an original birth certificate, a passport, or a certified birth card (issued by a health district). Photocopies will not be accepted. |
ADDRESS UPDATES: In order to ensure that your child’s mail is received by you (final report card, etc.), we need to have a current address on file. If you have moved recently, please call or stop by the registrar’s office as soon as possible to update this important information.
